As you consider your options, keep in mind the main difference is how you pay—either more up front (your rate per pay period) and less when you get care (deductible and copays) ... or less up front and more when you get care.

Pay period rates
The amount you pay per pay period depends on the plan you choose and the number of covered dependents. If your salary changes throughout the year, your pay period rate will not be impacted.
Understand our network tiers
Depending on which medical plan you choose, you'll have access to one or more provider networks.
Tier 1: BSW Premier
Best value!
The most cost-effective tier with the best value:
- 51,571 primary and specialty care physicians
- 379 hospitals and ambulatory surgical centers
- 300+ urgent care clinics
- 8,500+ behavioral health providers
- 12,000+ behavioral health providers through Lyra
Available with all medical plans and the only network for the SEQA and EQA plans (except in cases of urgent/emergency care or rare situations when an in-network provider is not available, and authorization is provided).
Tier 2: UnitedHealthcare
The next most cost-effective tier and only available with the Premium and HDHP plans:
- Nationwide coverage through UnitedHealthcare.
- If you or a covered dependent live 40 or more miles from the nearest Tier 1 acute-care hospital, you are eligible for the out-of-area benefit*. Care received by Tier 2 providers will be covered at the Tier 1 benefit level).
*This benefit must be activated through Baylor Scott & White Health Plan prior to receiving care. No action is required if previously activated.
Tier 3: Out-of-Network
The highest cost tier and only available with the Premium and HDHP plans.
- Includes physicians and providers, facilities and laboratories not included in any other network
Tobacco surcharge
We are committed to you and your family’s long-term health. If you and/or a dependent enroll in a medical plan, you will be asked about your tobacco usage within the last 12 months. If you confirm the use of tobacco products, you will pay a $30 tobacco surcharge for each member who uses tobacco, up to a maximum of $60 per pay period.
To remove the tobacco surcharge, you or your dependents must complete a tobacco cessation wellness coaching or have your physician provide a certification that you have quit using tobacco products. Please note that enrollment in the Tobacco Cessation Program will not prevent the tobacco surcharge from being applied. The surcharge will be applied until you or your dependents have completed the program and submitted the Tobacco Affidavit to PeoplePlace, which is provided once you complete the Tobacco Cessation Program.
